Retropharyngeal Dissection during Rubber Catheter-guided Nasotracheal Intubation: A Case Report

Abstract
The rare complication of retropharyngeal dissection (i.e., false passage of the endotracheal tube [ETT] into the nasopharyngeal mucosa) can occur during nasotracheal intubation (NTI). Use of a rubber catheter to guide the ETT during nasal passage is a modification of conventional NTI that is used to prevent retropharyngeal dissection. We present a case of inadvertent retropharyngeal dissection during rubber catheter-guided NTI performed under videolaryngoscopy. The retropharyngeal dissection was promptly recognized because a longitudinal submucosal bulge was visible through the videolaryngoscope. Inadvertent retropharyngeal dissection can occur during rubber catheter-guided NTI. When resistance is felt during passing of the ETT through the nasopharynx, an alternative method to overcome this resistance should be used. Prompt detection is vital to minimize damage.
